Nursing Services
Public Health Nurses serve New Haven’s culturally diverse urban community by promoting and encouraging a healthy lifestyle for its residents. They run the Health and Wellness Center, lead community health programs, and work inside New Haven Public Schools.
Parents and caregivers of Pre-K 3 & 4-year-olds:
All students should have received a seasonal flu vaccine by December 31, 2023.
Student Health Records
Student immunization records and Cumulative Health Records (CHR) are available upon request to students who go to or have graduated from New Haven Public Schools. Requests must be submitted using this form.
Any completed forms we receive will be processed within 7-10 business days.
School Requirements
Health information for school entry is based on state statutes and must be verified by a pediatrician (MD), advanced practice registered nurse (APRN), or Physician Assistant (PA) licensed to practice in Connecticut.
1. Physical exams / health assessments are required for students:
- Entering Preschool
- Entering Kindergarten
- Entering 7th grade
- Entering 10th grade
- Transferring into New Haven Public Schools
Fill out this digital form or provide a printed copy to the registration office or school nurse.
NOTE: if the exam takes place at a School Health Center, this form is not required.
2. Required Immunizations – Check this guide for required immunizations for students in Preschool through High School.
NOTE: CT State law no longer allows for Religious Exemptions for childhood immunization requirements for schools.
School Attendance Guidelines get updated based on latest medical advice.
- Children who have signs of communicable diseases should not be sent to school, until cleared by a healthcare provider.
- Parents should notify the school when their child is ill.
- If you’re having trouble deciding whether to keep your child home from school when ill, contact the school nurse’s office.
Addressing Student Health Needs
Communication is key!
It is important to notify your school nurse if your child has a food allergy, diabetes, asthma, other medical condition, or if ANY medication is needed during school hours. Specific forms may be required.
